General Liability Loss:

Auto - Bodily Injury/Property Damage Loss:

  • Third Party Contact Information Including Address & Phone Number
  • Description of Damaged Property or Bodily Injury

  • Any Witnesses

  Auto – Physical Damage Loss:

Worker’s Compensation Loss:

  • Date, Time and Exact location of Incident
  • Name, Addresses & Phone Numbers for All Involved Parties & Witnesses
  • What Work Was Being Performed by Employee at Time of Injury
  • Description of Occurrence and Injury
  • Injured Party’s Name, Address, Social Security Number, Phone Number, Date Hired, Occupation/Job title, Employment Status
  • Salary Per Week
  • What Emergency Care Was Provided
